Privacy Policy

This policy explains what Ishara Medical, LLC collects through this website, how that information is used, who it is shared with, and the choices you have — including how text messaging works and how to stop it.

Version 1.0 Effective 2026-01-01 Applies to https://www.isharallc.com
Draft for legal review. This policy was drafted from what this website actually does. Before publication, Ishara Medical's legal or compliance adviser should confirm every statement, remove anything that does not describe real practice, and add any notice required by the jurisdictions the practice operates in. Nothing here is legal advice, and publishing a privacy policy does not by itself make a website compliant with HIPAA or any other law.

1. Introduction

Ishara Medical, LLC ("Ishara Medical", "we", "us") provides advanced wound care and related services across Delaware and Maryland's Eastern Shore. This policy covers the information collected through this website — appointment requests, referrals, contact messages, newsletter signups and the consent choices you make on our forms.

Information that we hold about you as a patient, in the course of providing care, is handled under our clinical privacy practices rather than this website policy. Ask the practice for those if you need them.

2. Information we collect

We collect two kinds of information:

  • Information you give us when you complete a form on this site.
  • Limited technical information created automatically when you use the site, such as your browser type and, where we record a consent decision, the date and time and a shortened form of your IP address.
Review: the site currently stores a truncated IP address with consent records (configurable in includes/config.php). Confirm this is what the practice wants, and change the setting or this paragraph so the two match.

3. Information you provide

Depending on the form you use, that can include:

  • Name, phone number, email address, county and postal address
  • Date of birth and gender (referral form)
  • The name of a primary care physician or referring professional
  • Wound location and any comments you choose to write
  • Preferred appointment date, time and service
  • Your privacy and SMS consent choices, and the version of this policy in force when you made them

Please do not send detailed medical history, images or sensitive information through the website forms. Call the practice instead.

4. How we use information

  • To respond to your message or callback request
  • To schedule, confirm and coordinate appointments
  • To process and act on referrals
  • To send you the updates you subscribed to
  • To send text messages, if and only if you opted in
  • To keep a record of the consent you gave, so we can show what you agreed to and when
  • To operate, secure and improve the website

We do not sell your personal information.

5. Appointment information

When you request an appointment, the details you submit are emailed to the practice and a confirmation is emailed to you with a calendar invitation attached. An appointment request is not confirmed until a member of the team contacts you.

6. Contact information you send us

Messages sent through the contact form are delivered to the practice by email and used to reply to you. Email is not a secure channel; do not use it for anything you would not want read by someone other than the intended recipient.

7. SMS and text messaging

We send text messages only to people who have actively opted in by ticking the SMS box on one of our forms. The box is never pre-ticked, it is separate from the privacy acknowledgement, and:

  • Consent to receive text messages is not a condition of receiving services.
  • Message and data rates may apply.
  • Message frequency may vary.
  • You can reply STOP at any time to opt out, or HELP for assistance.

Messages relate to appointments, scheduling, care-related communications and other communications you asked for.

Review: this describes the consent capture that the website now performs. If a messaging provider is connected later, the provider's name, the message categories actually sent, and any carrier disclaimers required by that provider or by CTIA guidance should be added here.

8. Communication preferences

You choose how we contact you. If you do not opt in to text messages, we use phone or email. You can change your preferences at any time by calling (302) 568-7549, emailing info@isharallc.com, replying STOP to a text, or using the window on this site.

9. Information sharing

We share information only in these situations:

  • With clinicians and staff involved in your care
  • With a referring professional, where a referral has been made
  • With service providers who operate parts of our website, email or communications on our behalf, and only to do that work
  • Where required by law, legal process, or to protect someone's safety
  • With your direction or consent

We do not sell personal information, and we do not share it with third parties or affiliates for their own marketing or promotional purposes.

10. Service providers and technology providers

This website relies on a hosting provider, an email delivery path for form submissions, and a small number of third-party front-end libraries loaded from public content delivery networks (a calendar component, an icon set and web fonts). Those providers may receive technical information such as your IP address as a normal part of delivering files to your browser.

Review: list the actual hosting provider, email provider and any messaging provider by name here, and confirm whether a business associate agreement is required for each one.

11. Video consultation technology

This website does not currently provide video consultations, and no video conferencing provider receives information through it.

Review: the site owner's brief referenced Zoom guidance. Nothing in the website code uses Zoom, so this section says so. If Zoom is used by the practice for consultations or communications, set $uses_zoom = true; at the top of privacy-policy.php and have counsel write the accurate description, using the Zoom guidance supplied with the brief as the reference.

12. SMS and mobile information privacy

Mobile information is not shared for anyone else's marketing

No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Information sharing to subcontractors in support services, such as customer service or a messaging provider, is permitted. All other use case categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties.

In plain terms: your mobile number, the fact that you opted in, and the record of that consent sit apart from every other category of information sharing described in this policy. They are used to send you the messages you asked for, and to prove you asked for them. They are not passed to anyone else to market to you.

If you opt out, we keep a record of the opt-out itself. That is the only way to make sure no further messages are sent to you.

Review: this carve-out follows the standard structure messaging and campaign registries look for. Counsel should confirm the wording matches the practice's actual arrangements with any messaging provider before it is relied on.

13. Data security

We take reasonable measures to protect the information submitted through this site, including transport encryption, server-side validation of everything submitted, and restricted access to consent records. No website or email system is completely secure, and we cannot guarantee absolute security.

Review: confirm HTTPS is enforced site-wide, that the hosting account and database credentials are not shared, and that access to the consent records screen is behind real authentication before this paragraph is published.

14. Data retention

We keep form submissions and consent records for as long as needed for the purpose they were collected for, and for as long as required to meet legal, regulatory and record-keeping obligations. Consent and opt-out records are kept for as long as we may need to show what you agreed to.

Review: state the actual retention periods once the practice has decided them. A policy that says "as long as needed" and nothing else is weaker than one with real numbers.

15. Your privacy choices

  • Decline to provide information — though we may then be unable to schedule or respond
  • Opt in to, or out of, text messages at any time
  • Unsubscribe from email updates
  • Ask what we hold about you, ask for corrections, or ask us to delete information we are not required to keep

To exercise any of these, call (302) 568-7549 or email info@isharallc.com.

Review: the rights actually owed to a person depend on where they live and which laws apply to the practice. Counsel should confirm this list and add any jurisdiction-specific rights and response deadlines.

16. How to stop text messages

Reply STOP to any message from us and we will stop sending them. Reply HELP for assistance. You can also call (302) 568-7549 or email info@isharallc.com and ask to be opted out.

Opting out of text messages does not affect your care, and it does not stop us from contacting you by phone or email about an appointment you requested.

17. Cookies and analytics

This site uses a session cookie that keeps your form session valid and protects forms against cross-site request forgery. Your theme preference (light or dark) is stored in your own browser and never sent to us. No advertising or cross-site tracking cookies are used.

Review: if analytics or a tracking pixel is added later, set $uses_analytics = true; at the top of this file and update this section.

18. Children's privacy

This website is intended for adults. We do not knowingly collect information from children through it. Where a parent or guardian submits information about a child for care, that information is handled under our clinical privacy practices. If you believe a child has submitted information here, contact us and we will remove it.

19. Changes to this Privacy Policy

We may update this policy. Each published version carries a version number and effective date, shown at the top of this page. When you give consent on one of our forms, the version in force at that moment is recorded with your consent, so it is always clear which version you agreed to. The current version is 1.0.
